Development of health science popularization works for the prevention of deep vein thrombosis in patients after joint replacement

Objective To develop evidence-based health science popularization works for the prevention of deep vein thrombosis(DVT) in patients after joint replacement. Methods In this study,guided by the Guideline International Network patient guideline formulation manual and Patient Education Materials Assessment Tool,we conducted semi-structured interviews to clarify the health science themes. These themes were matched with relevant evidence,and the relevant evidence recommendation items for each theme were refined. It is translated into easily understandable language and other forms. 12 representatives(doctor,nurse,patient,science popularization expert and evidence-based methodology expert) conducted an external review of the quality of these science popularization works. Results Prevention of post-operative DVT,patients’ main concerns and the most urgent need for action were extracted from a total of 6 scientific topics. After the matching with 22 pieces of evidence,a health education content system for the prevention of DVT after total joint arthroplasty was created. According to the system framework and evidence,as well as the preferences and needs of patients,a popular science manual and popular science video were created,with the final external review rate of 94%~100% and the coefficient of variation of 0~0.101. The revised health science works for the prevention of DVT after joint replacement included popular science manuals-trifolds and popular science videos. The copyright of both popular science manuals(copyright number:Guozuo Dengzi-2022-L-10159159) and popular science videos(copyright number:Guozuo Dengzi-2022-I-10162339) have been registered. Conclusion The method of developing science popularization works on the prevention of DVT in patients after joint replacement is scientific. It can provide a methodological reference for the development of other science popularization works.

Objective To systematically review and synthesize the psychological experience of kinesiophobia in patients with cardiac rehabilitation. Methods PubMed,Web of science,Journals@Ovid,Embase,CINAHL,PsycINFO,Cochrane Library,CNKI,SinoMed,WanFang Database,Vip Database,American Heart Association,European Society of Cardiology and American Association of Cardiovascular and Pulmonary Rehabilitation were searched to collect qualitative research on the psychological experience of cardiac rehabilitation patients with kinesiophobia. The retrieval time was from the establishment of the databases to Jun 2023. The literature was evaluated using the Australian JBI Quality Evaluation Criteria for Qualitative Research in Evidence-based Health Care Centres(2016),and the results were consolidated using an aggregative integration approach. Results A total of 45 results were extracted from 14 studies. Similar results were summarized into 10 groups,and 3 integrated results were synthesized as followed. Kinesiophobia was influenced by many factors;kinesiophobia affects the life experience of patients;strategies to reduce the level of kinesiophobia. Conclusion Nurses should pay more attention to psychological experience of kinesiophobia,and take the corresponding intervention measures to help patients overcome the psychological barriers of kinesiophobia,perfect personalized exercise programs,and improve the level of physical activity.

Objective To systematically evaluate and integrate qualitative research related to the psychological experiences and perceptions of body image in burn patients. The goal is to provide a reference basis for developing targeted intervention measures in clinical practice. Methods The databases of CNKI,Wanfang,VIP Chinese Science and Technology Periodical Database,China Biomedical Literature Service System,PubMed,Web of Science,Embase,Medline,Cochrane Library,PsycINFO,and Scopus were applied in our study. Relevant qualitative studies on the body image experience of burn patients from the establishment of the databases until December 2022 were collected. The quality of the included literature was assessed using the 2016 edition of the Joanna Briggs Institute (JBI) Centre for Evidence-based Health Care in Australia,and the results of the literature were integrated using aggregative integration. Results A total of 12 pieces of literature were included,from which 40 research findings were extracted. These findings were ultimately summarized into 4 integrated results:diverse traumatic perceptions of body image changes faced by burn patients,active exploration and negative coping strategies towards body image changes faced by burn patients,emotional information needs and post-traumatic growth. Conclusion Burn patients experience various body image issues that hinder their return to normal life. It is crucial for healthcare professionals to timely attend to the physical and mental well-being of the patients,identify body image disorders,assist families in meeting emotional needs,support patients in self-adjustment,and promote positive outcomes.

Effectiveness and compliance of remote cardiac rehabilitation:an overview of systematic reviews

Objective To overview the systematic reviews of effectiveness and compliance of remote cardiac rehabilitation based on digital technology. Methods CNKI,Wanfang,CBM,Cochrane Library,JBI,BMJ,PubMed,Embase,CINAHL,PROSPERO databases were searched to identify the relevant systematic reviews. The retrieval period was from February 2012 to October 2022,and 2 researchers screened systematic reviews and extracted the data independently. Results A total of 14 systematic reviews were included. The evaluation results showed that the quality level of 2 systematic reviews was medium,and 6 systematic reviews were at low level,and 6 were at very low level. The meta-analysis results show that the application of digital technology,including communication technology,APP,virtual reality,wearable devices and other remote assistance measures can effectively improve patients’ cardiopulmonary function,exercise ability and quality of life. However,the effect of virtual reality technology needs to be further verified. Conclusion Remote assistance can improve the effectiveness and compliance of cardiac rehabilitation,but the methodological quality of systematic evaluation and the quality of evidence are low,and most of them are based on foreign research. Localization debugging is needed when the evidence is applied,and high-quality research in line with Chinese national conditions should be carried out.

Relational model of factors influencing fluid management in hemodialysis patients based on the health belief model

Objective To investigate the influencing factors and pathways of fluid management adherence and interdialytic weight control in hemodialysis patients based on the health belief model. Methods Using convenience sampling,433 patients receiving hemodialysis were included from 4 hospitals in Guangdong province. The demographic and clinical data questionnaire,Health Belief Scale of Fluid Restriction,and Fluid Intake Adherence Scale were used. The relative-interdialytic weight gain(R-IDWG) was calculated. Results The mean score of patients’ fluid management adherence was 3.08±0.97,indicating a moderate level,with 35.57% of patients exceeding the required R-IDWG standard. Regarding health beliefs,perceived benefits(β=0.161) and self-efficacy(β=0.685) of fluid restriction directly affected fluid adherence;perceived threats of poor fluid restriction indirectly affected fluid management adherence through perceptions of benefits and self-efficacy(β=0.235);perceived barriers were indirectly and negatively associated with fluid management adherence through self-efficacy(β=-0.246). Higher fluid management adherence was associated with better control of R-IDWG(β=-0.361). Regarding the coordinating factors,age,educational level,hemodialysis vintage and diabetes were influential factors of fluid management adherence(|β|:0.050-0.162) and R-IDWG control(|β|:0.049-0.309). The coefficients(β) of each path were statistically significant(P<0.05). Conclusion Healthcare providers should comprehensively assess patients’ health beliefs,focus on patients with lower educational levels and diabetes,and identify weakness in their fluid management to achieve the required R-IDWG standard.

To summarize the nursing experience of a patient with diabetic nephropathy who underwent xenogeneic islet transplantation after kidney transplantation. The key points of nursing mainly include:establishing a multidisciplinary diagnosis and treatment team before surgery,improving relevant examinations and preparations,and ensuring the safety of surgery;postoperative dynamic anticoagulant therapy and bleeding observation and nursing to prevent rejection and infection. The goal-oriented blood glucose management was implemented in the whole process,and systematic health education and personalized psychological nursing were carried out.After careful specialized nursing,the patient recovered on the 26th day of hospitalization and was discharged from hospital. Up to now,the patient has completely separated from exogenous insulin treatment.

Objective To systematically evaluate and integrate qualitative studies on the experience and needs of telerehabilitation for patients with cardiovascular diseases,so as to provide references for clinical practice in China. Methods We searched databases including Cochrane Library,JBI,PubMed,Embase,Web of Science,Psyc INFO,CINAHL,CBM,WanFang Data,CNKI and VIP from inception to February 2023,to collect qualitative studies related to the experience and needs of telerehabilitation for patients with cardiovascular diseases. The quality of included studies was evaluated according to JBI Critical Appraisal Tool for qualitative studies in Australia. Meta-synthesis was used to integrate results. Results A total of 15 studies were included;65 findings were collected;the similar results were grouped into 11 new categories;finally these categories were integrated into 4 synthesis results,including acceptance of telerehabilitation in patients with cardiovascular disease,perceived benefits of telerehabilitation in patients with cardiovascular disease,barriers to telerehabilitation in patients with cardiovascular diseases,telerehabilitation needs and expectations of patients with cardiovascular disease. Conclusion As a convenient alternative to telerehabilitation additional cardiac rehabilitation,telerehabilitation has been recognized by most patients with cardiovascular diseases. However,future research should continue to optimize digital tools,improve rehabilitation models,strengthen team building,provide social support,and improve the accessibility and experience of cardiovascular disease patients using telerehabilitation systems.

Objective To explore the barriers in the clinical use of evidence-based deep vein thrombosis prevention after joint replacement,and to provide bases for clinical measures. Methods The interview outline was developed based on the Theoretical Domains Framework(TDF) from July to August 2020. Totally 16 nurses working in the department of orthopedic at a tertiary A hospital in Beijing were selected for semi-structured interviews by convenient sampling,and the content analysis was conducted based on the results. The content of the transcribed text is analyzed,and specific topics and codes are classified according to the relevant fields of TDF. Results In clinical practice,the department has used the HIS system to assess DVT risks,but the nurses do not have a good grasp of the assessment scale,and the case information need to be retrieved,which has caused certain obstacles to the development of the assessment work. Nurses have the awareness to carry out health education and nursing prevention,but due to the lack of auxiliary tools,it is difficult to attract patients’ attention,and thus they lose the will to act,and hold doubts about the prevention effects. Nurses master health education related content and can implement physical and drug prevention,and the department has carried out prevention effect evaluation. However,due to the lack of relevant procedures,there are differences in behaviors among nurses. Combined with TDF,the above obstacles are classified into 5 domains:knowledge,environmental factor,motivation and target,result expectation,and code of conduct. Conclusion There is a variety of obstacles that affect the prevention of deep vein thrombosis after joint replacement in clinical application. It is necessary for the department to carry out the reform of evidence-based nursing practice on the basis of obstacles and promote the clinical application of the best evidence for the prevention of deep vein thrombosis after joint replacement.

Objective To carry out evidence-based nursing for the prevention of deep vein thrombosis(DVT) in patients after joint replacement,to formulate review indicators,to analyze the obstacles and promoting factors in the practice of evidence-based nursing,and to formulate action strategies. Methods Based on the Knowledge to Action framework as the theoretical guidance,a total of 24 pieces of evidence for DVT prevention nursing in patients after joint replacement in suitable clinical situations were included;6 examination criteria and 33 examination indexes were formulated;the preventive behavior of nurses was observed on-site with patients as a unit. Furthermore,based on the results of the baseline review,brainstorming was used to analyze the barriers and facilitators in evidence-based practice,and action strategies were formulated. Results Totally 24 pieces of best evidence were included. According to the best evidence,33 examination indexes and 6 examination criteria were established. The clinical compliance rate of the 6 examination criteria ranges from 0% to 100%. Among them,there is room for improvement in health education(0),mechanical prevention(62%),pharmacological prevention(81%) and quality control(21%). Obstacle factors include the lack of tools and nursing processes,lack of knowledge,insufficient attention,busy work,as well as low understanding and low participation. Facilitating factors include strong organizational leadership,medical team cooperation,sufficient equipment,etc. Accordingly,corresponding action strategies have been formulated. Conclusion Based on evidence and clinical experience,we formulated the scientific,appropriate and maneuverable indicators. At present,there is a certain gap between the evidence for the prevention of deep vein thrombosis in patients after joint replacement,and we should fully utilize these facilitating factors,overcome obstacles,implement improving measures,and promote the effective application of evidence.

Construction and application of a volume management program in vulnerable phase for patients with heart failure

Objective To construct a volume management program based on Information-Knowledge-Attitude-Practice(IKAP) theory and to discuss its application effect for patients with heart failure in vulnerable phase. Methods The 4 process and capacity management architecture based on IKAP theory were constructed by the expert group discussion method. Using the convenience sampling method,94 patients with heart failure from May to October 2021 were selected as the experimental group,and the same number of patients from May to October 2020 were selected as the control group,both of whom were admitted to a tertiary first-class hospital in the city of Nanchang. The experimental group received the volume management program based on IKAP theory,and the control group received the routine care of the department. Finally,we evaluated the effect of the intervention. Results 94 patients in the experimental group and 92 patients in the control group were finally collected. There were statistically significant differences in self-care ability,weight management ability,B-type natriuretic peptide level,dry body mass compliance rate and readmission rate between 2 groups after the intervention(P<0.05). Conclusion The volume management program based on IKAP theory can improve self-care ability,weight management ability and dry body mass compliance rate of vulnerable patients with heart failure,and reduce B-type natriuretic peptide level and readmission rate in vulnerable phase.

Design and application of newborn transport thermal clothes

Objective To investigate the design and clinical application of transport thermal clothes in neonates. Methods The clothing is made up of 2 parts,including a cotton-padded coat and polyethylene plastic film. A total of 174 premature infants admitted to the NICU of a tertiary hospital in Suzhou from September to December 2020 were selected as the research subjects. The experimental group used the self-designed transport thermal clothes for intrahospital transport,while the control group used the traditional cotton swaddling clothes for intrahospital transport. The temperature,blood sugar and blood gas analysis of the 2 groups of premature infants were observed after they entered the NICU. Results The body temperature of the experimental group into NICU was(36.45±0.19) ℃,while it was(36.38±0.15) ℃ in the control group(t=-2.57,P=0.01).The blood glucose in the experimental group was (3.33±0.97) mmol/L,and it was(2.07±0.95) mmol/L in the control group(t=-8.63,P<0.05). The lacacid was (1.58±0.63) mmol/L in the experimental group and(1.97±1.45) mmol/L in the control group(t=2.29,P=0.02). Conclusion The newborn transport thermal clothes is easy to operate,and it can increase body temperature of premature during the administration process,ensure the stability of blood sugar. It can also reduce the lactic acid level.

Influence of blood pressure fluctuation on operative side palm swelling in patients with radial artery compression after percutaneous coronary intervention

Objective To investigate the relationship between the changes of blood pressure and palmar swelling during radial artery compression in patients with radial artery percutaneous coronary intervention (PCI). Methods The patients who underwent radial artery PCI in the coronary heart disease unit of a tertiary general hospital in Nanchang from May 2018 to December 2020 were selected as the study subjects. The general data and the related indexes of decompression were analyzed retrospectively,and the blood pressure fluctuation during radial artery compression was described. Univariate regression analysis was used to analyze the influencing factors of palmar swelling on the operative side,and the blood pressure during radial artery compression was divided into nodes. The effects of different blood pressure on operative side palm swelling were analyzed by stratification,and the corresponding range of blood pressure at the highest risk of operation side palm swelling was further analyzed by trend regression. Results A total of 788 patients with radial artery PCI were included. Based on the analysis of patients’ baseline data,it was found that history of diabetes,history of hypertension,signs of heart failure and high average systolic blood pressure during radial artery compression were the risk factors of operative palmar swelling. However,multivariate stratified analysis of risk factors showed that systolic blood pressure ≥130 mmHg during radial artery compression increased the risk of ipsilateral palm swelling in patients with diabetic history (OR=1.09,P<0.001). In patients with or without heart failure signs and hypertension history,when the systolic blood pressure during radial artery compression was more than 130 mmHg,the 2 groups increased the risk of palmar swelling on the operative side. Furthermore,trend regression analysis showed that the average systolic blood pressure during radial artery compression was between 130 and 149 mmHg. Every 10 mmHg increase in systolic blood pressure had increased the risk of palmar swelling by 3.52 times[95%CI (1.37,13.22) ]. Conclusion During the period of radial artery compression,the average systolic blood pressure of patients is 130-149 mmHg,and the risk of palmar swelling on the operative side is the highest. It is suggested that clinical nurses should comprehensively consider the changes of systolic blood pressure when formulating the prevention plan for complications of palmar swelling on the operative side.

Analysis of nurses work status and differences between secondary and tertiary hospitals in Guangdong Province

Objective To describe the work status of nurses in secondary and tertiary hospitals in Guangdong province in China,and to analyze the differences between secondary and tertiary hospitals. Methods A total of 4,163 nurses from 36 secondary and tertiary hospitals were investigated with questionnaires on the demographic characteristics,nursing work environments,care left undone,burnout,job satisfaction and intention to stay. Results The overall score of nursing work environments was 3.03(2.74,3.61);the score:for the subscale of staffing and resources adequacy and nurse participating in hospital affairs were the lowest,which were 3.00(2.50,3.50) and 3.00(2.56,3.56),respectively. Perception of nurses in tertiary hospitals on nursing work environments was significantly worse than that of nurses in secondary hospitals for both overall scale and 5 subscales(all P<0.001). The average number of care left undone reported by nurses was 6.0(1.0,11.0),with a significantly higher number of those reported by nurses in tertiary hospitals than those in secondary hospitals (P=0.036). Nurses in the secondary and tertiary hospitals experienced a moderate level of emotional exhaustion and depersonalization,and a high level of reduced personal accomplishment. Furthermore,61.42% of nurses were satisfied with their work and 79.87% had the intention to stay. Compared to nurses in secondary hospitals,the level of burnout among nurses in tertiary hospitals was at a higher level(P<0.001 both for emotional exhaustion and depersonalization),and the proportion of nurses who were satisfied with their work and willing to stay was lower(all P<0.050). Conclusion In Guangdong province,attention should be paid to further improvement of the work environment. Due to heavy workload and low job satisfaction of nurses,this situation was more prominent in tertiary hospitals than it in secondary hospitals.

The outbreak of coronavirus disease 2019 brought new challenge to the hospital management. Nursing staff is the cornerstone to fight against the epidemics. It is vital to allocate and manage the nursing staff effectively and reasonably for prevention and control of the infectious disease. This article describes the construction of the management system,the assessment and allocation of nurses,the training and examining and performance management in a tertiary hospital in western China. The purpose of our study is to discuss the ideas and methods of the nursing staff management in the face of sudden epidemics,and to provide references and experience to manage nursing human resource effectively.

Objective To investigate and analyze the consistency between self-reports and nursing notes in adjuvant chemotherapy-related side effects among patients with breast cancer. Methods During July 2017 to July 2018,a total of 128 patients with first-time adjuvant chemotherapy after breast cancer surgery who were treated in a tertiary cancer hospital in Jiangsu Province were enrolled by convenience sampling method. Patients were investigated by general information questionnaire and self-reported questionnaire about chemotherapy-related side effects of breast cancer. At the same time,side effects information was also extracted from the electronic nursing notes to be compared with patient-reported data. Results The frequency and severity of chemotherapy-related side effects were consistently greater in patient-reported data than nurse-noted data(P<0.05). In addition to the side effect of “vomiting”,the consistency between patients and nurses was general,the consistency of other items was low. Conclusion Nurses tended to underreport or underestimate side effects,including several symptoms that may cause serious complications. In the future,patient-reported outcomes should be considered for integration into clinical practice in order to facilitate patient participation in decision-making and improve the medical experience.

Effects of resistance training on bone mineral density in patients with postmenopausal osteopenia or osteoporosis:a meta-analysis

Objective To identify the effects of resistance training on bone mineral density in patients with postmenopausal osteopenia or osteoporosis using meta-analysis.Methods We performed a systematic search in PubMed,CINAHL,Embase,Web of Science,Wanfang,CNKI and CBM to collect studies about effects of resistance training on bone mineral density in patients with postmenopausal osteopenia or osteoporosis published from inception to April 2019. Meta-analysis was performed using RevMan 5.3 software after two independent reviewers conducted independent literature screening,data extraction,and literature quality evaluation according to inclusion and exclusion criteria.Results Seven studies were recruited,including 698 cases. The results of meta-analysis showed that resistance training had a positive effect on bone mineral density at the lumbar spine in patients with postmenopausal osteopenia or osteoporosis compared with conventional exercise. The combined effect was statistically significant[MD=0.02,95%CI(0.00,0.05),P=0.04]. Resistance training had no significant effect on bone mineral density at the femoral neck[MD=0.01,95%CI(-0.00,0.02),P=0.17],hip[MD=0.01,95%CI(-0.01,0.02),P=0.41],greater tro-chanter[MD=0.01,95%CI(-0.00,0.03),P=0.16]and serum osteocalcin[MD=1.44.95%CI(-2.23,5.12),P=0.44].Conclusion Resistance training significantly improved bone mineral density in the lumbar spine,but did not improve bone mineral density in the femoral neck,hip,greater trochanter and serum osteocalcin in patients with postmenopausal osteopenia or osteoporosis. At present,there is insufficient evidence for the effects of resistance training on bone mineral density in patients with postmenopausal osteopenia or osteoporosis,the sample size varies greatly,and more high-quality studies are needed for future research.
